If the Hiss is not expelled after this period, the changes affect the genetic material and are irreversible, they irrevocably losing control of their bodies. During this period the mind of the infected experiences a nightmare whose content seems to depend on the personality and experiences of the infected, in Jesse's case it was an officemare, but in this period the changes are reversible and the Hiss can be entirely expelled from the body and mind without it takes damage, like Jesse did.
